The ICS-213 General Message Form is the standard for incident messaging, all DRT members should keep the various formats of this document on their computers and keep a printed supply in their go-kits.

This .pdf format is suitable for printing blank forms to give to agency representatives. When they give you a message to relay, you should assign the message a message number and write it in the upper right hand corner of the page for fast and easy reference. NOTE: See Annex D -- Digital Communications of the DRT Ops Manual for information on tactical message numbering.
This MS-Word document (.doc) allows an operator to fill in the appropriate fields then print out the completed message. NOTE: This document is protected (Read Only) -- be sure to assign the message a message number and save it to disk before printing and sending. There is no field specifically for the message number, recommend you make it the first item in the message block.
For digital communications (packet and HF digital) we have two ASCII versions of the ICS-213. Digital operators should keep both these forms on their laptop computers and as bulletins on their TNC mailbox so they are readily available. The first version is for use in actual incidents (emergency operations).
NOTE: To download these text files, left click the link and the
document should open in your browser. From the menu bar, use File>Save
as>filename.ext to save it to disk (if you right click the link
and use
"Save Target as" the file formatting will be for UNIX).
The second version should be used during exercises. This version contains the phrase "EXERCISE MESSAGE" both before and after the message to prevent anyone monitoring from acccidentally mistaking exercise messages for actual emergency messages.
You should keep both these forms on both your computer. Digital
operators should NOT -- UNDER ANY CIRCUMSTANCES -- send the large
MS-Word document over the air. All digital operators should use
the .txt versions for messaging in both incidents and exercises.
